The application form is available for download during the month of April. One must fill the application form online and complete the formalities of registration. This registration must carry out, before the last date of registration mentioned by the AFMC (usually given in May 2025). The registration process requires a candidate to check the login for the UG link and then click the new user icon. This opens up the data form with mandatory fields to be filled in capital letters. The candidates require to fill up personal details like email addresses, dates of birth, etc, accurately and without fail before proceeding to the next page. Once registered, the candidate receives the password to the email address registered. It is used to login and fills in the qualification details for the application form.
Online documents that need uploading include the photograph of the candidate and his/her signature. Both the documents should not exceed the 500KB limit. The application fee for the application is Rs 250 which can be remit through SBI Bank Challan.

The age limit for candidates applying for the AFMC 2025, falls between 17 years to 22 years of age as of December 2025. That is, the candidate must be born not prior to January 01 1999 and not later than December 31, 2000.
Mandatory educational qualifications required include, the candidate having passed Mathematics in the secondary board examination. He/She must pass the higher secondary or 10+2 or any equivalent exam covering Physics, Biology, Chemistry, and English, in his/her first attempt. The aggregate marks should not be less than 60%. Depending upon the AIPMT score, eligible candidates shortlist to appear for ToELR and psychological assessment interview which is held during June or July at the AFMC, Pune institute. The ToELR is a computer-based test that consists of 40 multiple choice questions, each carrying 2 marks. There is a negative marking of 0.5 for every wrong answer. The duration for answering all the 40 questions in 30 minutes.
The maximum mark adding to the 720 of the AIPMT is 800 for which the candidates evaluate. This score is then divided by 4, to arrive at a total score of 200 marks. The interview awards a maximum of 50 marks which constitutes the total maximum score of 250.
